National Case Management Week is October 13-19. The American Case Management Association emphasizes the impact professionals in case management make every day. Across all care settings, case managers are making a difference!

National Case Management Week

This year’s theme is Case Management: Setting New Standards for Care, created by the American Case Management Association (“ACMA”). The purpose of this week is to recognize the compassion, dedication, and quality patient results put forth by professionals in case management. The ACMA encourages everyone to recognize all case managers for their tireless and exceptional efforts in achieving quality patient results. Working across all home care agencies, assisted living facilities, hospitals, and other healthcare organizations, we celebrate the incredible difference that case managers make in the lives of their patients.

ACM Recognition Day: Wednesday, October 16th

In line with the National Case Management Week, Wednesday, October 16th, is ACM Recognition Day, the perfect chance to congratulate the case managers who have earned their Accredited Case Manager (ACM) certification and support those who are in the process of becoming ACM-certified. Many organizations, like Assisting Hands Richmond Home Care, choose this day to celebrate with the ACM professionals, congratulating and rewarding them to honor this  important industry specialization. At this time, the ACMA encourages home care agencies, assisted living facilities, hospitals, and other health care organizations to recognize and honor the importance of case management professionals as they dedicate their careers to serving patients and their families.

Honoring Case Managers for Their Work

Now in 2019, the ACMA’s Fundraising Committee’s recognition initiative has returned. In honor of National Case Management Week, this program works to provide case management professionals with an opportunity to provide a gift that honors an individual, chapter, department, or organization involved in health care. We encourage case managers to remember and honor those who have made an impact in their life and career as well as those who make a difference in the lives of patients and families they serve. When making your donation, your individual or organization’s name will appear in ACMA’s briefCASE e-newsletter as well as on the ACMA website in October 2019. All honorees will also appear in the Fall issue of Collaborative Case Management Journal.

About Case Management

If you are unfamiliar with case management, it is a collaborative practice in hospitals, rehabilitation facilities and other healthcare organizations that includes physicians, social workers, patients, nurses, other practitioners, and caregivers in the community. It encompasses communication and regulates care through effective resource coordination. The goal of all case management professionals is the achievement of optimal health, appropriate utilization of resources, and full access to care.

About ACMA

The ACMA was founded in 1999 as a national, non-profit, membership association that dedicates itself towards providing solutions, support, and resources to Case Management and Transitions of Care professionals. The organization is comprised of over 8,000 members across the U.S. and includes physicians, social workers, nurses, and other professionals affiliated with case management.
