You are recovering and on your way home. Yay!! Assisting Hands® Home Care can help guide you through the complicated process of getting home safely and continuing your recovery in the comfort of your own home. Our goal is to ensure you remain at home and are not readmitted to the hospital.

We do this by carefully planning your transition from the hospital or rehab center and your continued care at home. A report by the CDC states that nearly 20% of people aged 65 and over are readmitted back to the hospital within 30 days.

Four main reasons for readmission are:

  1. Lack of Follow Up: more than half of patients who are readmitted within 30 days did not follow up with their primary care doctor.
  2. Lack of Medication Management: 75% of patients fail to take their medications correctly due to the inability to understand the directions, remember a new medication schedule, or discontinue medications that have been changed.
  3. Lack of Communication: More than 50% of patients do not communicate changes in their condition to their primary care doctor. What could be a simple office visit ends up being a readmission.
  4. Living Alone: A person that lives alone has a 50% or higher chance of being readmitted. This is mostly due to a lack of assistance in the home to pick up food, medications and assist with follow up appointments.

Assisting Hands Home Care caregivers will:

  • Coordinate with your discharge team to ensure proper follow up
  • Do a Home Safety Evaluation
  • Ensure prescriptions are provided and get New Prescriptions
  • Provide Medication Reminders and remove old or unnecessary meds
  • Schedule follow up doctor appointments and help you get to them
  • Assist with the use of new equipment (wheelchair, walker, Hoyer, etc.)
  • Prevent Falls and provide, Awareness, and Education
  • Help with Illness/Recovery Specific Education
  • Monitor Change in Condition and Communicate with Primary Doctor or Nurse
  • Assist with Bathing, Grooming, Dressing or Toileting
  • Provide Meal preparation and Proper Nutrition
  • Do Light Housekeeping/Laundry
  • Provide Companionship/Supervision for Safety and Peace of Mind
